1. Download your APPLICATION in WORD (docx) or PDF (pdf) format. 
2. Email the completed application with a copy of your license and a copy of your pay stub to gowens@fulcrumpmgmt.com. 
3. Submit the Application Processing Fee by clicking on the APPLICATION FEE button below.
4. Allow 3 - 4 business days for processing.
